COMPLAINT FORM

Based on the questionable expenditure of taxpayer funds in connection with attendance of town officials at a February 2005 conference held in NYC, the undersigned hereby requests an investigation of the Towns records of this matter be initiated. If allegations are sustained, appropriate action should be initiated to recover improperly spent funds and to discipline or remove from office those responsible officials.
